Doctors warn: swine flu vaccine poses too many risks

MARK COLVIN: The country's top infectious diseases body has written to the Government to warn that the swine flu vaccine is being distributed too hastily, with too many risks for the public.
The Australasian Society for Infectious Diseases says the vaccine will come in multi-dose vials.
It says these have been shown to transmit infection, spread HIV and hepatitis and even cause death.
The Federal Health Minister Nicola Roxon says the Government expects to receive two million doses by the end of next week, and a widespread immunisation program could start by mid-September.
But the infectious diseases doctors say the swine flu threat has now passed, and there's no need for urgent mass vaccination.

DINA ROSENDORFF: It's unusual to hear a doctor speak out like this:
TOM GOTTLIEB: To rush to a massive vaccination using multi-dose vials that have been associated with problems in the past, seems too hasty and perhaps not measured enough for our society.
DINA ROSENDORFF: Dr Tom Gottlieb is the President of the Australasian Society for Infectious Diseases, which represents about 400 clinicians nationally.
The doctors are worried about the method that will be used to deliver the swine flu vaccine, when the national immunisation program starts in the next few weeks.
The plan is to use multi-dose vials of the vaccine. That means a health worker will use needles to take more than one dose out of each vial.
TOM GOTTLIEB: There have been episodes of bacterial contamination of vaccines, where there has been perhaps a bacterial organism growing in the vaccine, and when the patient's been injected they may have become infected with a bacteria.
Now that's largely been addressed with preservatives that are used in vaccines, but nonetheless, there are potentials for viruses from one patient to be transmitted if vaccination is not done correctly. And in most circumstances it would be done correctly, but when things are rolled out hastily and multi-dose vials are used that potential exists and there are certainly examples of spread of infection between patients with HIV, hepatitis B, hepatitis C and so on and that's our concern.
DINA ROSENDORFF: The group of infectious diseases doctors is so concerned that they've written to Australia's Chief Medical Officer, Jim Bishop, urging him to reconsider the vaccination strategy.
In the letter, the doctors point out that many international agencies including The World Health Organisation prefer the use of single-dose vials.
But the drug company making the vaccine says it would take too long to make enough single-dose vials.
Dr Rachel David is a spokeswoman for CSL.
RACHEL DAVID: It would delay the role out of the vaccine by a long time. The other reason is that with pre-filled syringes you need to order a lot of syringes to roll out the vaccine for 21 million doses as the Government has ordered. So I think on balance the decision to use multi-dose vials is completely justified.
DINA ROSENDORFF: Is it a cost saving measure?
RACHEL DAVID: Look, although it will work out being cheaper to use multi-dose vials, it's not being done this way to save money. It's been done this way to vaccinate a large number of people quickly, which is our best chance of stopping the flu epidemic.
DINA ROSENDORFF: But Dr Gottlieb says there's no real need to rush.
He says there have been fewer deaths linked to swine flu than initially feared, and the epidemic has now subsided.
TOM GOTTLIEB: Our society, all the members are strong advocates of vaccinations, so in no ways do we want to undermine vaccination as a very effective strategy. But in this particular influenza season, we have to determine which are the risk groups that need vaccination, and the probable urgency is no longer there as it was perhaps a few months ago. And we would caution for a more measured approach and for single dose vaccines to be used preferably to multi-dose vials.
DINA ROSENDORFF: But that argument has done nothing to change the mind of Australia's Chief Medical Officer.
Professor Jim Bishop is yet to sign off on the swine flu vaccine rollout, but he's indicated that when he does, multi-dose vials will be used.
In a statement, he's told PM that health workers will be trained to maximise infection control and minimise wastage.
The Federal Health Minister, Nicola Roxon, says the Government is likely to take delivery of two million doses by the end of next week.
NICOLA ROXON: It's anticipated that by the middle or end of September we would be starting to vaccinate the priority groups and those priority groups are of course the most vulnerable that we would hope to be able to reach as quickly as possible. And we are certainly doing everything we can to make sure that the vaccine is available and that implementation plans are ready to go as soon as we do get that tick off.
DINA ROSENDORFF: The first jabs will go to pregnant women, health workers and people with underlying medical conditions.

Man with AR-15 rifle at Obama rally sparks concerns

A man carrying an AR-15 rifle outside the Phoenix Convention Center where President Barack Obama was speaking today has renewed concerns about protesters bringing firearms to presidential events.
"It is extremely disturbing that you have that kind of weapon in close proximity to where the president is," said Ruben Gallego, a military veteran and Arizona Democratic Party official who observed the man.
"He was demonstrating his Second Amendment rights," Gallego added, "but he was clearly there to intimidate people who were there exercising their First Amendment rights."
Neatly dressed in a white shirt, black tie and gray slacks, the man, who only gave his first name as Chris, also had a pistol holstered at his side as he engaged in heated debates with those rallying in support of Obama's heath-care reform plan.
A Phoenix police spokesman said plainclothes detectives were monitoring about a dozen protesters carrying guns, though no one broke any laws or was arrested.
Arizona is an "open-carry" state, which means anyone legally allowed to have a firearm can carry it in public as long as it's visible. A permit is required if the weapon is carried concealed.
"Because I can do it," Chris said when asked why he brought guns to the rally at 3rd and Washington streets. "In Arizona, I still have some freedoms left."
Gallego, who served a tour of duty in Iraq, said he believes the AR-15 semi-automatic rifle was loaded. He spotted a magazine in the firearm and another in the man's back pocket.
Obama was inside the downtown convention center, addressing a national gathering of the Veterans of Foreign War. The president's motorcade entered and exited the facility about a block away from where protesters and supporters were staged.
"We're here to keep the peace. If we need to intervene, we will intevene at that time," said police Detective J. Oliver, one of three officers shadowing the man with the rifle.
Last week, during Obama's health-care town hall in Portsmouth, N.H., a man with a pistol strapped to his leg held a sign reading: "It is time to water the tree of liberty." It was a reference to Thomas Jefferson's quote, “The tree of liberty must be refreshed from time to time, with the blood of patriots and tyrants.”
Earlier at the event, another man was arrested for having an unlicensed gun in his car at the school where Obama spoke.
In response to the armed demonstrators in Phoenix, Paul Helmke, president of the Brady Campaign to Prevent Gun Violence, called on the National Rifle Association and other gun-rights groups to tell their members to leave their weapons at home when attending presidential events.
"Individuals carrying loaded weapons at these events require constant attention from police and Secret Service officers, thus stretching their protective efforts even thinner," Helmke said.
"The possibility that these weapons might be grabbed or stolen or accidentally mishandled increases the risks of serious injury or death to all in attendance."
Calls to several NRA officials in Arizona were not returned.

PHOENIX, Arizona (CNN) — He was to demonstrating his right to bear arms — and he wanted you to know it.
Video of the unidentified man toting an assault rifle outside President Obama’s speech to veterans Monday was aired all over the country, causing a buzz about weapons popping up — legally — around recent presidential events.
The protester, who refused to give his name, was interviewed by a man carrying a microphone and said, "I am almost always armed."
The interview, done by Libertarian radio talk show host Ernest Hancock, was staged.
"Absolutely," Hancock told CNN’s Rick Sanchez Tuesday. "You guys are so easy. What we wanted to do was make sure that people around the country knew that law enforcement in Phoenix, Arizona, protects our rights."
The Phoenix-based host of "Declare Your Independence with Ernest Hancock" identified the bespectacled man with the AR-15 semi-automatic assault rifle as "Chris," saying he’s known him for two years as part of a younger generation of Libertarians.
"We are up against a tyrannical government that will rob the next generation as long as they can get away with it," Hancock said…
And while Hancock admitted the interview was staged, he insisted the protester’s message was not.
"I come from another state where ‘open carry’ is legal, but no one does it, so the police don’t really know about it and they harass people, arrest people falsely," the protester identified as Chris told Hancock in an interview aired by CNN affiliate KNXV. "I think that people need to get out and do it more so that they get kind of conditioned to it."
Video from KNXV shows the man standing with other protesters, with the rifle slung over his right shoulder, a handgun in a holster on his left hip, and a bullet clip in his back pocket.
The same protester told KNXV separately, "I’m exercising my rights as an American in Arizona."
The protester was among a dozen other demonstrators carrying unconcealed guns outside the Obama event.
Hancock, who said he was packing a 9-millimeter Beretta pistol, added that Phoenix police had known about the group’s intent to protest while carrying guns…
Arizona law has nothing in the books regulating assault rifles, and only requires permits for carrying concealed weapons. So despite the man’s proximity to the president, there were no charges or arrests to be made, according to Phoenix police…
Phoenix police spokesman Sgt. Andy Hill on Monday said officers explained the law to some people who were upset about the presence of weapons at the protest…
Hill told CNN on Tuesday that the widely broadcast protest was no different than any other for law enforcement, in that investigators with the "community response unit" had communicated with the group ahead of time…
He added that because of Arizona’s open carry law, it’s not uncommon for protesters to pack heat at demonstrations.
Gun-toting protesters have demonstrated around the president before. Last week, a man protesting outside Obama’s town hall meeting in New Hampshire had a gun strapped to his thigh. That state also doesn’t require a license for open carry…
Asked whether the individuals carrying weapons jeopardized the safety of the president, Donovan said, "Of course not." …
Of course CNN is being just a little disingenuous here. This wasn’t "staged" in the sense that it was pretending to be something it was not.
The "bespectacled" protester is a real libertarian who believes in our Second Amendment rights and brought his gun to protest Mr. Obama’s anti-gun policies.
The only thing that could be conceivably considered "staged" is that the libertarian radio talk show host knew about him, and knew he was going to interview him about the gun. That is, the radio host didn’t just happen to bump into a total stranger at the protest.
That is not "staged" in the sense we usually mean when talking about such things.
Fortunately for us, our media never stages such things.
Though for some odd reason CNN and the rest of our watchdog media seemed determined not to give us a full look of ‘Chris’ the protester in any of their still photographs.
(The photo at the top, for instance, is from our screen grab from the local station’s interview.)
Instead, in every still photograph our media masters seemed to prefer just to show the rifle, while artfully cropping the man’s face.
You would never know the protester was a clean cut (and articulate) black man.
That’s what we call “staged.”

Cap-and-Trade's Unlikely Critics: Its Creators

Journalists Show Bias on Global Warming Issues
Their choice of words and the structure of these comments disclose their preference and prejudices of the global warming issues. They also show a remarkable lack of what science is and how it proceeds. These are the self-appointed journalists who view themselves as final arbiters and reporters of what science is. They haven’t a clue.
What is remarkable is that a society of Journalists, allegedly intending to honestly discuss complex issues, has forfeited their professed dispassionate indifference to the global warming issues in exchange for naked advocacy.

One man in the audience who asked a number of questions asked that in light of the problems connected with the 1976 swine flu vaccine – Guillain-Barre syndrome – “why would I be in favor of this?”
Thurman replied that Guillain-Barre syndrome is a rare neurological condition that causes temporary paralysis. But that the methods used to create the current vaccine have been different since the early 1980’s.
The same man then joked, “It’s like that monkey movie … you know, 12 Monkeys, where they guy flew around the world in a plane spreading that disease around the world.”
Strangely, no one mentioned anything about the current swine-flu outbreak having to do anything with a man-made release.

Because H1N1 flu is a novel virus, parents are concerned as 12,000 children in Oklahoma are going to be tested with vaccinations for H1N1 to see whether there are any serious side effects. Some parents are complaining the only benefit is going to be for the vaccine manufacturers.
Among the parents' fears are the fear of autism, fear of children becoming paralyzed by novel reactions similar to the syndrome that occured with a different type of swine flu vaccine in 1976.
The problem is that the onset of serious side effects could take four weeks or longer to appear.
US Hospitals to use children to TEST swine flu vaccine
Children’s Mercy will test swine flu vaccine on 120 kids. Testing of swine flu vaccine on children could begin next week at Children’s Mercy Hospital and other medical centers nationwide.
Unless studies under way on adults turn up unexpected side effects, Children’s Mercy anticipates receiving the vaccine early in the week. Children 6 months to 17 years old are eligible for the test vaccine for the H1N1 virus. The vaccine will be given to 120 children.
Public distribution of the vaccine could start in October.

Link between swine flu jab and deadly syndrome will be probed

The Health Protection Agency (HPA) has asked doctors to check for increases in a brain disorder called Guillain-Barré syndrome (GBS) once the national vaccination programme begins.
Its letter refers to the use of a swine flu vaccine in the United States in 1976, when 25 people died from GBS, while just one died from swine flu.
The syndrome, which can be fatal, attacks the lining of the nerves, causing paralysis and inability to breathe. Concerns have already been raised that the new vaccine has not been sufficiently tested and that the effects, especially on children, are unknown.
The jabs being developed by pharmaceutical companies and will be given to about 13 million people during the first wave of the programme, expected to start in October.
Priority will be given to everyone aged six months to 65 with an underlying health problem, pregnant women and health professionals.
In a letter sent by the HPA on July 29, neurologists have been asked to monitor closely any cases of GBS as the vaccine is rolled out.
It alerts them to the use of a swine flu vaccine in the US in 1976, which was followed by more than 500 cases of GBS, including 25 deaths. The US programme was stopped after 10 weeks because of concerns over its safety, and the Government paid out millions of dollars in compensation to those affected.
The swine flu virus in the new vaccine is a different strain from the 1976 virus, but the possibility of an increased incidence of GBS remains a concern.
According to the Mail on Sunday, two letters were posted together to neurologists advising them of the concerns. The first, dated July 29, was written by Professor Elizabeth Miller, head of the HPA's Immunisation Department.
It says: "The vaccines used to combat an expected swine influenza pandemic in 1976 were shown to be associated with GBS and were withdrawn from use.
"GBS has been identified as a condition needing enhanced surveillance when the swine flu vaccines are rolled out.
"Reporting every case of GBS irrespective of vaccination or disease history is essential for conducting robust epidemiological analyses capable of identifying whether there is an increased risk of GBS in defined time periods after vaccination, or after influenza itself, compared with the background risk."
A second letter from the Association of British Neurologists is written by Dr Rustam Al-Shahi Salman, chairman of its surveillance unit (BNSU), and Professor Patrick Chinnery, chairman of its clinical research committee.
It says: "Traditionally, the BNSU has monitored rare diseases for long periods of time. However, the swine influenza (H1N1) pandemic has overtaken us and we need every member's involvement with a new BNSU survey of Guillain-Barré syndrome that will start on August 1 and run for approximately nine months.
"Following the 1976 programme of vaccination against swine influenza in the US, a retrospective study found a possible eight-fold increase in the incidence of GBS. Active prospective ascertainment of every case of GBS in the UK is required. Please tell BNSU about every case."
Last night, the HPA insisted that it did not expect to find links between the new vaccine and GBS, and said it wanted to monitor any connection between the flu virus itself and the syndrome.
Professor Miller at the HPA said: "This monitoring system activates pandemic plans that have been in place for a number of years. We'll be able to get information on whether a patient has had a prior influenza illness and will look at whether influenza itself is linked to GBS.
"We are not expecting a link to the vaccine but a link to disease, which would make having the vaccine even more important."
The UK's medicines watchdog, the Medicines and Healthcare Products Regulatory Agency, is already monitoring reported side effects from Tamiflu and Relenza and it is set to extend that surveillance to the vaccine.
A Department of Health spokesman said: "The European Medicines Agency has strict processes in place for licensing pandemic vaccines.
"In preparing for a pandemic, appropriate trials to assess safety and the immune responses have been carried out on vaccines very similar to the swine flu vaccine. The vaccines have been shown to have a good safety profile.
"It is extremely irresponsible to suggest that the UK would use a vaccine without careful consideration of safety issues. The UK has one of the most successful immunisation programmes in the world."
